Registration

The Conference Registration Form is now available!

A Mail-In Registration Form is available for you to print, complete and mail with your payment. Print and complete one copy of the form for each attendee.

NOTE: If you plan to attend the conference but have not registered yet, please send an email to one of the following conference staff members so they can be sure to have adequate supplies available.

The registration form is in pdf format which requires Adobe Reader to open.

Online registration is no longer available.

You must make you own hotel reservations.  The conference hotel is:

They are holding a limited block of single and double rooms at the conference rate of $99/night plus tax.

 Rooms in the held block that have not been booked will be released on May 10^th.
